INFORMATION
Medical Release Forms Your team may not practice, play a game or hold any team function without the medical release forms. If you hold any team function without the medical release forms you may be held personally liable if any situation arises. You should make a copy of the medical release forms and have the parents sign them again, (The form must have a "wet signature") and give them to your assistant coach so that the both of you have them. Practices Carry your field permits with you at all times. This is the only proof that you have that you can practice at the fields if any other organized group or city official questions why you are there. Teams may not practice without adult supervision. All "helpers" must fill out and submit a current volunteer form available at easyo.org. Once this is done they must e mail Alicia Wood, Region 780 Child Protection Volunteer Advocate, to let her know that the application has been filled out and she will give them their volunteer ID number. (Two copies of the application must be signed and submitted to Alicia or Cruzanne.) They then must complete their Safe Haven training before they can help at practices. Only AYSO registered players may practice with your team. Siblings, friends, etc. are not allowed to practice with your team. If a player from another team is practicing with your team you must have their medical release form. Shin Guards are mandatory for the protection of the player. A player must wear shin guards at all practices and games or they will not be allowed to participate. Practice on the grass. As you know the fields are not in the best conditions and we want to limit the damage as much as possible. Stay off the dirt areas. GAMES Games are to be played according to the schedule. Do not assume that a game is canceled due to rain as that may determined shortly before the start of the game at the game site. You will be contacted as soon as possible if your game has been canceled. Make sure that we have current cell phone number to contact you or the person who you designate as the team contact. You are responsible for contacting all of your players if the game is canceled. Each region makes their own decision regarding cancellations. If you travel, make sure that you return you signed line up card to the ref table by the following weekend. Goal Set Up, Take Down and Field Maintenance If you have the first game of the day you have goal set up. If you are playing against another Region 780 team, you will be responsible for one goal. If we are hosting a team you will be responsible for the set up of both goals. If you have the last game of the day you will need to take down the goals. All goals with the exception of the U6 goals come apart. Carefully take them apart and place them neatly in the container. The U6 goals are not taken apart. They are screwed together. Just carry them to the container and place them inside the container to the left of the other goals. They are placed next to each other not on top of one another. After your game make sure that the field area is picked up of all trash
and left neat for the next game. Have your parents move back from the
field so that the next games fans may set up their chairs for their game.
