Join the fun, Fall Season 2008, register your child in the world's most popular sport "SOCCER".

WE NEED VOLUNTEERS SO THE KIDS CAN PLAY. SIGN UP TO BE A COACH, REFEREE, ETC. TRAINING PROVIDED
Volunteers must register by August 1, 2008 at www.eAYSO.org

Please note: Your child is not register until you #1 Register online, #2 Come to Registration Location, #3 Sign all 3 copies of forms, and #4 Provide Payment

STEP #1: Register online
PRE-REGISTER: www.eAYSO.org
Print 3 copies of registration formand bring them to any registration date listed above. (If the player has never been registered with AYSO, a copy of the players' birth certificate must be brought to registration.)

STEP #2: Come to Registration Location

STEP #3: Sign Registration Forms
Registration forms MUST be signed by Parent / Guardian AT REGISTRATION LOCATION.

STEP #4: Provide Payment at a Registration Sign-up Location

Payment: (cash or personal check* accepted)
Registration Fee: $90.00 (cash or personal checks* accepted)
Multi Family Member Discount: $85.00 for additional child

*Checks returned by Bank will be assessed a $20.00 Fee

Registration Fee includes uniform (jersey, shorts and socks), team and individual picture, AYSO National registration fee, and supplemental accident insurance.

FOR SAFETY AND LEGAL REASONS ALL REGISTRATION FORMS MUST BE COMPLETELY FILLED OUT AND PROCESSED BEFORE PLAYERS WILL BE PLACED ON A TEAM.
